Role Overview
We are seeking an experienced Regional Patient Care Liaison to support strategic referral relationships, census growth, and market development across our Skilled Nursing Facilities in the Tennessee region. This role combines direct relationship development with hospitals and referral partners along with regional mentorship and coaching of Patient Care Liaisons.
What You Will Do
Conduct frequent in-person outreach with key referral partners, support facilities in strengthening their hospital partnerships, and provide hands-on coaching through ride-a longs and field mentoring to develop high-performing liaison teams.

Requirements
- 3+ years of healthcare business development experience
- Experience in Skilled Nursing, Post-Acute Care, or Healthcare Services
- Demonstrated success in new business development and relationship management
- Experience supporting strategic referral accounts and hospital partnerships
- Ability to mentor, coach, and develop business development staff
- Strong communication, relationship-building, and presentation skills
- Ability to travel frequently throughout the Tennessee region
- Valid driver’s license
